Step #1: Insurance

On the back of your insurance card is an 800 number. Call the number to learn about the behavioral health benefits available to your specific policy.
- What is the extent of my behavioral health coverage?
- Do I need to get pre-authorization for behavioral health treatment?
- What is the deductible of my plan and is there a co-pay or coinsurance due at the time of each appointment?
- Does my policy cover the Turning Point Evaluation, INC provider I have selected?
Step #2: Choose a Therapy
On this webpage you will note different target areas of therapy or counseling under Treatment Services. You may click on the service in which it will then give you a brief explanation where you can learn about the focus areas. If you prefer, you can call our office at 515.462.5967, and our administrative staff can help you choose a provider.
Step #3: Make an Appointment
Call our office at 515.462.5967. Our administrative staff will help you schedule your first appointment and confirm your insurance information. Our clinicians maintain their own schedules, we will try to accommodate with your schedule in order to best service your needs.
Step #4: Patient Forms
The initial paperwork required takes about 30 minutes to complete. On the day of your initial appointment, please arrive early enough time to complete the forms.
Step #5: Your First Appointment
Please arrive 30 minutes prior to your first appointment time in order to fill out the required paperwork.
Please do not forget to bring:
- Your insurance card
- A photo ID
- If court ordered, please bring the case ID number.
Check in with the front desk. Our administrative staff will make a photocopy of your insurance card, your photo ID and review your information.
